On Wednesday 13th September 2017 East Devon Health held a conference with national attendees to showcase the strategic approach to workring together in healthcare. There is now increasingly seen to be a gap in the provision of routine healthcare for residents in care homes. There is a largely shared view that the current model for providing primary care to care homes can be suboptimal and haphazard. This may lead to fragmentation of care, and crisis intervention rather than preventative management.
